Description of Services: Hospital-based outpatient day program with a focus on geriatric rehabilitation * services include:

  • discharge planning
  • individual and group exercises
  • health education
  • medical and nursing assessments

  • occupational therapy

Treatment is offered in three-hour sessions twice a week for 12 weeks

Eligibility - Population(s) Served: Adults 65 years and older (55 years and older in special circumstances) who will benefit from two or more program disciplines * must be able and willing to participate in a group setting * must be able to toilet independently * must be able to learn and fellow recommendations delivered in English, or be accompanied by someone who can translate * must have ongoing and reliable transportation to and from program
