Eligibility - Population(s) Served: | Adults over the age of 65 with multiple medical, functional, or psychosocial problems and unable to attend appointments onsite. Clients must reside within the catchment area of Keele Street to Yonge Street and Steeles Avenue to St. Clair Avenue | ||
Application: | Referral from a physician or nurse practitioner is required for a geriatric assessment and should be initiated through our Central Navigation. | ||
