Admission Criteria
Age Range: 65 years and older
Diagnosis: balance issues, dialysis, difficulty walking, falls, mild cognitive impairment, Parkinson's disease, stroke, diabetes, aphasia, swallowing difficulties, frailty, deconditioning, failure to thrive
Other:
Exclusion Criteria
Medical: patients requiring more than minimal assistance for transfers/ambulance * cognitive difficulties preventing patient participation
Other: patients able to receive services in a long-term care home or retirement residents * patients requiring only one service * previous admission to Geriatric Day Hospital in last 2 years with no significant change in status
