Medical Stability: | must be medically stable | ||
Rehabilitation Potential: | must be able to identify rehab goals for functional improvement for one or more of the following services: occupational therapy, physiotherapy, speech language pathology, social work | ||
Age Range: | 18 years and older | ||
Diagnosis: | confirmed diagnosis of a recent stroke (up to two years post) | ||
Behaviour: | does not demonstrate behaviours that would pose risk to self or other | ||
Participation: | able to participate in at least 30 minutes of therapy per therapeutic service needed * demonstrates ability and willingness to engage in new learning, and to practice learned strategies in everyday life | ||
Function: | can manage independent toileting, or can be reliably accompanied by someone who can provide assistance | ||
Other: | have reliable transportation to and from rehab |
