Service is for Individuals Who: have a confirmed diagnosis of a recent stroke (up to two years post)

Admission Criteria
Medical Stability: must be medically stable
Rehabilitation Potential: must be able to identify rehab goals for functional improvement for one or more of the following services: occupational therapy, physiotherapy, speech language pathology, social work
Age Range: 18 years and older
Diagnosis: confirmed diagnosis of a recent stroke (up to two years post)
Behaviour: does not demonstrate behaviours that would pose risk to self or other
Participation: able to participate in at least 30 minutes of therapy per therapeutic service needed * demonstrates ability and willingness to engage in new learning, and to practice learned strategies in everyday life
Function: can manage independent toileting, or can be reliably accompanied by someone who can provide assistance
Other: have reliable transportation to and from rehab
