Description of Services: Programs and services for adults dealing with face-to-face or written communication challenges as a result of acquired brain injury, stroke, or Parkinson's disease * services include:

  • communication assessment
  • training and education about devices
  • assistance purchasing or providing communication devices

Progression and Maintenance levels of rehabilitative care for patients working to restore or maximize functionality, or prevent functional decline or injury, or to maintain functionality
Requirements
Fees: Set fees * publicly-funded services available to those who are eligible * funding may be available through the Assistive Devices Program or third-party insurance
Eligibility - Population(s) Served: Adults 18 years and older whose speech or language does not meet their daily communication needs, or are physically unable to produce functional written communication * must be able to attend appointments at Bridgepoint * must be able to identify a facilitator who will support their communication system (e.g. scheduling appointments, completing paperwork, basic troubleshooting)
Application: Medical referral required

Inpatients - Consult doctor or care team for an internal referral

Outpatients - Complete the GTA Ambulatory Services referral form and the Applicant Information form

Referral forms can be submitted by fax or mail
