Description of Services: Interprofessional program that involves an in-home visit from an occupational therapist and pharmacist to help older adults reduce their risk of falls * licensed therapists conduct in-home safety assessment and recommends assistive devices or home modifications * pharmacists educate on proper use of medications and prescriptions * offers six-week group exercise and education program to help improve range of motion, strength, and balance

Progression and maintenance levels of rehabilitative care for patients working to restore or maximize functionality, or prevent functional decline or injury, or to maintain functionality * physiotherapy program takes place at the St John's Rehab Campus, starting with an assessment to determine individual needs

Eligibility - Population(s) Served: Medically stable adults 65 years and older * must be cognitively alert, oriented and able to follow instructions * must have family/caregivers to attend assessments if there are language barriers or other communication difficulties
