Program: Chronic Disease - Central West Self-Management Program

Description of Services: Through a partnership with the Central West Self-Management Program (CWSMP), Osler provides support for people with chronic health conditions, including diabetes * workshops are group-based and non-clinical

Peer-led group sessions include chronic disease self-management, diabetes self-management and chronic pain self-management. Each six-week session runs for two and a half hours, once a week

Topics include:

  • setting personal goals 
  • creating realistic action plans 
  • identifying and solving common problems 
  • managing pain and fatigue 
  • developing healthy eating and physical activity plans 
  • managing depression, stress and difficult emotions 
  • managing medications 
  • working with health care providers 
  • planning for the future

At this time, sessions are offered online only through Zoom to residents of the Central West region (Brampton, Etobicoke, Dufferin-County, Caledon, Malton, Woodbridge and Rexdale) * Online programs can be brought to community groups and organization
