Description of Services: Specialized program providing assessment, treatment and care for patients with multiple chronic complex medical conditions and diagnoses. The interdisciplinary team supports and optimizes patient-centred care and goals focusing on quality of life. *Priority will be given to patients with complex medical needs (as listed below).

Eligibility - Population(s) Served: Medically stable patients 60 years and older with chronic complex illness with multiple co-morbidities that require ongoing medical intervention

Service is for Individuals Who: have chronic complex illness with multiple co-morbidities that require ongoing arrangements

Average Length of Stay: length of stay will vary by individual needs, determined by the health care team
Admission Criteria
Medical Stability: must be medically stable
Age Range: 60 years and older
Diagnosis: Have chronic complex illness with multiple co-morbidities that require ongoing medical management.
Other: if patient smokes, must be able to do so safely and make own arrangements
Exclusion Criteria
Medical: Dialysis * mechanical ventilation * Bi-Pap * cuffed tracheostomy tube * needs greater than 50% oxygen * Total parenteral nutrition (TPN) * stage 1 and 2 ulcers without complex medical needs * Patients with a gastrostomy tube or well established tracheostomy without complex medical needs * patient requires extensive rehabilitation or physiotherapy
Bariatric: bariatric equipments needs (300lbs+)
Behaviour: Patients with responsive behaviours will be assessed on a case by case basis
Other: patient's needs can be reasonably met in a community or long-term care setting * patient requires a locked/secure unit

Respiratory Care Oxygen: Tracheostomy
IV Therapy: PICC
Nutrition: GJ Tubes
Skin Condition: Specialized Wound Care
Continence: Indwelling Urinary Catheters
