Admission Criteria | |||
Medical Stability: | must be medically stable | ||
Rehabilitation Potential: | has restorative potential and may attain a maximum level of functioning given patient's medical condition | ||
Age Range: | 55 years and older | ||
Participation: | has the mental, physical, and cognitive endurance to participate at the minimum level * able to participate in at least 60 minutes of activity several times per day for High Tolerance Short Duration and 30 minutes daily for Low Tolerance Long Duration | ||
Function: | has sitting tolerance and able to sit up-right for at least one hour | ||
Patient Goals: | has identified goals that are specific, measurable, realistic, and timely | ||
Other: | has established realistic and appropriate discharge plans to facilitate return to community living * expectation that discharge planning and discharge destination are determined prior to admission | ||
Exclusion Criteria | |||
Medical: | Recent stroke or acquired brain injury* mechanical ventilation *BiPAP *tracheostomy tube *Oxygen requirements greater than 50% *TPN *Vacuum-Assisted Closure(VAC) therapy- this would be evaluated on a case-by-case basis *weight bearing status <50% for both upper and lower extremities, treatment for other co-morbid illnesses/conditions that interfere with the patient's ability to participate in rehab (e.g. dialysis or active cancer treatment) and elective surgeries for hip and knee replacement (No Bundle care approach). | ||
Bariatric: | Bariatric equipment needs (300 lbs+) This would be evaluated on a case-by -case basis | ||
Behaviour: | Unmanaged responsive behaviours that limit patient's ability to participate at minimum level required |
