Description of Services: Assessment and treatment of chronic pain * services provided by doctors, nurses, occupational therapists, physical therapists, and pharmacists * after first visit, patient's treatment plan may include:
  • medication changes
  • injections
  • occupational therapy
  • physical therapy
  • cognitive therapy
  • referral to a self-management program
