Eligibility - Population(s) Served: Adults 65 years and older who have experienced changes in their memory, thinking, mood, personality and/or behaviour; a recent decline in ability to manage daily activities; recent falls or changes in balance or mobility; multiple medications and/or multiple chronic health conditions; repeated visits to the emergency department; or explained weight loss or new incontinence
